The smartest timing is after the airline, flight number and hotel are settled but before check-in opens. Dominican rules require airlines to verify that passengers have completed the form, which explains why a gate agent in the United States or Canada may ask for the QR before anyone in Punta Cana does.
One charge does belong to the trip: the $10 Dominican tourist card. For travelers arriving by air, both the airport and the U.S. State Department say it is included in airline charges. Check with the carrier if your itinerary is unusual, but do not pay a second person at PUJ simply because they call the eTicket a tourist card.

Confirm the departure eTicket and save its QR. PUJ recommends arriving at least three hours before US-bound flights because extra screening can apply. Include the drive from your actual resort area, not a generic Punta Cana estimate.
